Carleton University Protocols

As previously communicated Carleton University will be requiring mandatory vaccination against COVID-19 for all individuals that are eligible under Ottawa Public Health's vaccination age guidelines (12 years of age or older as of December 31, 2021) wishing to access athletics facilities for any amount of time, for any reason - indoors or outdoors.

As a result, all individuals must complete the COVID-19 screening self-assessment tool before coming to campus as well as attest to their vaccination status and having received their first dose by September 8, 2021, and their second dose by no later than October 15, 2021 using cuScreen.


cuScreen is a web portal that enables visitors to upload proof of vaccination as well as complete the required COVID-19 screening self-assessment to generate their self-assessment status before coming to campus and entering athletics facilities. The self-assessment status will indicate whether an individual should come on campus (green status) or stay at home (red status).


To create your cuScreen account click here. A separate cuScreen account is required for each individual 12 years of age or older as of December 31, 2021.  If asked, the code is: CARLETONU-VISITOR.

Step 1: Report Your Vaccination Status

  • Log into cuScreen
  • Click on "Report your latest vaccination status"
  • Carefully read each question and answer appropriately
  • Upload proof of vaccination (vaccine dose administration receipts or equivalent documents), preferably in PDF format

If you have only received one dose of a two dose vaccine series, you can complete your vaccination attestation form with one dose now and update your form 14 days after you received your second dose.

Vaccination dose administration receipts are available from most government websites. Individuals who received their vaccine in Ontario can download their proof of vaccination through Ontario's COVID-19 vaccination website. Individuals who received their vaccine in Quebec can download their proof of vaccine through the Government of Quebec's website.

Any false information or misrepresentation on the mandatory attestation form is a violation of university policy that will be taken very seriously.

Step 2: Complete Your COVID-19 Screening Self-Assessment before coming to Campus

  • Log into cuScreen
  • Click on "Complete your Self-Assessment"
  • Carefully read each question and answer appropriately
  • Click on “Submit”
  • If you receive a “red status” stay at home
  • Scan QR Code upon arrival at building entrance

Exemption Requests

Any individual who cannot be vaccinated on medical grounds or other protected grounds recognized by the Ontario Human Rights Code can request an exemption. Unvaccinated individuals without an approved permitted exemption will not be able to attend campus or any University activity in-person.
